Current Openings

Registered Nurse (RN) - Home Health

Full-time

Provide skilled nursing care to patients in their homes throughout Chicago and surrounding suburbs. Conduct assessments, develop care plans, and coordinate with physicians and the interdisciplinary team.

Requirements:

  • Current Illinois RN license
  • Minimum 1 year of home health or relevant nursing experience
  • Valid driver's license and reliable transportation
  • Strong assessment and communication skills
  • BLS certification

Physical Therapist (PT) - Home Health

Full-time / Part-time

Deliver physical therapy services to patients in their homes, helping them regain mobility, strength, and independence. Develop individualized treatment plans and work closely with patients and families.

Requirements:

  • Current Illinois PT license
  • 1+ years of experience preferred
  • Valid driver's license and reliable transportation
  • Excellent interpersonal and motivational skills

Occupational Therapist (OT)

Full-time

Help patients develop, recover, improve, and maintain the skills needed for daily living and working. Provide adaptive equipment recommendations and caregiver training.

Requirements:

  • Current Illinois OT license
  • Experience in home health or geriatrics preferred
  • Valid driver's license and reliable transportation
  • Creativity and adaptability in treatment approaches

Speech-Language Pathologist

Part-time / PRN

Evaluate and treat patients with speech, language, voice, and fluency disorders. Provide dysphagia management and cognitive-communication therapy in home settings.

Requirements:

  • Current Illinois SLP license
  • CCC-SLP preferred
  • Experience with adult and geriatric populations
  • Valid driver's license and reliable transportation

Home Health Aide (HHA)

Full-time / Part-time

Provide personal care and assistance to patients in their homes. Help with activities of daily living while providing companionship and emotional support.

Requirements:

  • Illinois HHA certification
  • CPR certification
  • Compassionate, reliable, and patient-centered approach
  • Valid driver's license and reliable transportation
