Registered Nurse (RN) - Home Health
Full-timeProvide skilled nursing care to patients in their homes throughout Chicago and surrounding suburbs. Conduct assessments, develop care plans, and coordinate with physicians and the interdisciplinary team.
Requirements:
- Current Illinois RN license
- Minimum 1 year of home health or relevant nursing experience
- Valid driver's license and reliable transportation
- Strong assessment and communication skills
- BLS certification
Physical Therapist (PT) - Home Health
Full-time / Part-timeDeliver physical therapy services to patients in their homes, helping them regain mobility, strength, and independence. Develop individualized treatment plans and work closely with patients and families.
Requirements:
- Current Illinois PT license
- 1+ years of experience preferred
- Valid driver's license and reliable transportation
- Excellent interpersonal and motivational skills
Occupational Therapist (OT)
Full-timeHelp patients develop, recover, improve, and maintain the skills needed for daily living and working. Provide adaptive equipment recommendations and caregiver training.
Requirements:
- Current Illinois OT license
- Experience in home health or geriatrics preferred
- Valid driver's license and reliable transportation

Speech-Language Pathologist
Part-time / PRNEvaluate and treat patients with speech, language, voice, and fluency disorders. Provide dysphagia management and cognitive-communication therapy in home settings.
Requirements:
- Current Illinois SLP license
- CCC-SLP preferred
- Experience with adult and geriatric populations
- Valid driver's license and reliable transportation
Home Health Aide (HHA)
Full-time / Part-timeProvide personal care and assistance to patients in their homes. Help with activities of daily living while providing companionship and emotional support.
Requirements:
- Illinois HHA certification
- CPR certification
- Compassionate, reliable, and patient-centered approach
