VA Community Care Network
(VACCN) Explained
At HomeHealth, Inc. is a fully credentialed VACCN provider — helping Illinois veterans access the home health care they've earned, through the VA Community Care Network, with costs that may vary based on your individual VA benefits and service-connected disability status.
The VA Community Care Network
The VA Community Care Network (VACCN) is a program created by the Department of Veterans Affairs that allows eligible veterans to receive health care services from approved community providers — like At HomeHealth, Inc. — outside of VA facilities.
Through VACCN, veterans can receive skilled nursing, therapy, and other home health services at home, in their own community, billed directly to the VA. Your out-of-pocket costs, if any, depend on your VA service-connected disability rating and the specific services authorized. Contact your VA care team for your individual cost details.
- Administered by Optum on behalf of the VA
- Covers skilled nursing, physical, occupational and speech therapy, medical social work, and more
- Costs vary by service-connected disability status — contact your VA team for your specific cost share
- At HomeHealth, Inc. handles all billing directly with the VA on your behalf
- We are in-network for every Illinois VA facility
