VA Authorized Provider In-Network All Illinois VA Facilities

VA Community Care Network
(VACCN) Explained

At HomeHealth, Inc. is a fully credentialed VACCN provider — helping Illinois veterans access the home health care they've earned, through the VA Community Care Network, with costs that may vary based on your individual VA benefits and service-connected disability status.

What Is VACCN?

The VA Community Care Network

The VA Community Care Network (VACCN) is a program created by the Department of Veterans Affairs that allows eligible veterans to receive health care services from approved community providers — like At HomeHealth, Inc. — outside of VA facilities.

Through VACCN, veterans can receive skilled nursing, therapy, and other home health services at home, in their own community, billed directly to the VA. Your out-of-pocket costs, if any, depend on your VA service-connected disability rating and the specific services authorized. Contact your VA care team for your individual cost details.

  • Administered by Optum on behalf of the VA
  • Covers skilled nursing, physical, occupational and speech therapy, medical social work, and more
  • Costs vary by service-connected disability status — contact your VA team for your specific cost share
  • At HomeHealth, Inc. handles all billing directly with the VA on your behalf
  • We are in-network for every Illinois VA facility

How VACCN Home Health Care Works

Contact Us First

Call or email At HomeHealth. We'll confirm your eligibility and explain the full VACCN process — at no charge.

VA Authorization

Your VA primary care provider submits a referral. The VA approves the care and issues an authorization to At HomeHealth through VACCN.

Plan of Care

We develop an individualized care plan in coordination with your VA care team — tailored to your specific health needs and goals.

Care Begins at Home

Our skilled clinicians come directly to your home. We bill the VA directly. Your cost share, if any, is determined by your VA benefits and service-connected disability status.

Who Qualifies

VACCN Eligibility for Veterans

Most veterans enrolled in VA health care may qualify for Community Care services. The VA considers several factors when determining eligibility. Contact us and we can help you determine if you or a loved one qualifies.

  • Enrolled in VA health care
  • VA cannot provide the needed service in a timely manner
  • VA facility is more than 30 or 60 minutes away (drive time standards)
  • Care is a VA-covered benefit
  • Referred by a VA primary care provider

Services Covered Under VACCN Home Health

  • Skilled Nursing (wound care, medication management)
  • Physical Therapy
  • Occupational Therapy
  • Speech-Language Pathology
  • Medical Social Work services
  • Disease management and care coordination
Note: All VACCN services require prior authorization from the VA. At HomeHealth, Inc. works directly with your VA care team to obtain and manage all authorizations on your behalf.
